2. Cheque payment by post
Complete the application form and post it together with a cheque made payable to " JJ Nel " to  46 Atney Rd, Putney, London, UK, SW15.

IMPORTANT REQUEST
Please, under no circumstances, e-mail a scanned copy of this form. Follow the instructions below to complete and submit the form via e-mail:
1) Right click on the Order Form form link below
2) Click on "Save Target As"
3) Select the folder in which to save the file on your Hard disk from the "Save in" box
4) Type the file name in the "File Name" box. The file name should be labelled in the following     manner: "surname_initials_order.rtf" (Example: Jeans_M_order.rtf)
5) Click "Save"
6) Open the saved file, fill in the form and post as and attachment to uk_clivia@yahoo.co.uk
7) If appropriate, give details of bank where cheque was paid in, and date of payment.
