The annual subscription for UK members can be confirmed by contacting Jaco Nel, and can be paid by a sterling cheque drawn on a UK bank. Payment of subscriptions should be made by one of the following two methods listed below.

Please, under no circumstances, e-mail a scanned copy of this form, follow the instructions below to complete and submit the form via e-mail:
1) Right Click on the Membership form link below
2) Click on "Save Target As"
3) Select the folder in which to save the file on your Hard disk from the "Save in" box
4) Type the file name in the "File Name" box. The file name should be labelled in the following manner: "surname_initials_member.rtf" (Example: Jeans_M_member.rtf)
5) Click "Save"
6) Open the saved file, fill in the form and post as an attachment to uk_clivia@yahoo.co.uk
7) If appropriate, give details of bank where cheque was paid in, and date of payment.
