Publications Order Form
IMPORTANT INFORMATION
* This form should not be used by members resident in Australia and Australasia , United Kingdom , North and South America, Canada and Mexico and New Zealand.
IMPORTANT REQUEST
Please, under no circumstances, e-mail a scanned copy of this form, follow the instructions below to complete and submit the form via e-mail:
1) Right Click on the Publications Order form link below
2) Click on "Save Target As"
3) Select the folder in which to save the file on your Hard disk from the "Save in" box
4) Type the file name in the "File Name" box. The file name should be labelled in the following manner: "surname_initials_order.rtf" (Example: Jeans_M_order.rtf)
5) Click "Save"
6) Open the saved file, fill in the form and post as an attachment to publications@cliviasociety.org
